Please choose one option. Nothing is preselected. Choosing no photo or video permission has no effect on a place. The young person can decline to be photographed at any time.

Typed confirmation of permission

Type both full names exactly as entered in step 1. Your acknowledgements, the permission wording, programme details and submission time will be kept with the application. If the host requires a handwritten signature, the team will arrange it separately.

A place is reserved only when your reference appears.

Opening or completing the form does not hold a space. We can accept a maximum of 20 students. Keep your reference and contact the team for changes or withdrawal. No email or SMS confirmation is sent by this form.
